As part of your consultation I will typically ask for information such as name, address, date of birth, email address, telephone number and personal information about your health both past and present, your current and past medication and the health of your family. This information will be used in your consultation and stored safely to support any future consultations with you.
The information you provide is confidential and will not be shared with anyone else unless obliged to by law, or it is felt that you are at risk of harming yourself or someone else.
Herbal Practitioners can be subject to practitioner supervision and support. Where this happens, all information will be anonymised, but your health condition may be discussed in order to ensure I continue to provide the best patient care and outcomes.
Unless you explicitly ask for your data to be deleted, information provided a part of a consultation will be retained and stored safely for at least 7 years. Any children’s data will be held until the patient is at least 25 years old. This complies with legal obligations, can be used to resolve any possible disputes, and aligns normal good practice.
Finally, this website is hosted on Squarespace, Inc. (Squarespace). Any information entered via this website uses Squarespace’s data storage, database and the general Squarespace application.
